Palliative Care

Liberty HomeCare & Hospice Services provides palliative care to help patients enjoy life and daily activities without unnecessary discomfort. Palliative care reduces or alleviates the pain inherent with a serious and chronic illness. Liberty can provide this care to anyone at any stage of an illness, and can administer palliative care simultaneously with other treatments and therapies.

Typical diagnoses prompting palliative care include cancer, cardiac disease, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), neuromuscular diseases (such as Parkinson’s), Alzheimer’s Disease, HIV/AIDS, Amyotrophic Lateral Sclerosis (ALS), scleroderma and renal failure.

When ordered through Liberty HomeCare & Hospice Services, palliative medications will be coordinated by a multidisciplinary team of physicians, nurses, specialists and social workers, and carefully monitored for effectiveness and optimal dosages.
